Please contact our office for more detailed information about rates and fees. We offer a sliding scale for families and individuals who can provide financial eligibility through declaration of income.

Out of network: While we don’t accept most insurance plans, we help clients submit directly to their insurance company. After payment of our service and upon completion of testing, we will provide an itemized invoice to submit to your insurance carrier for reimbursement.

We accept personal check, money orders, or credit card payments. 50% of payment is due prior to rendering of services.

Cancellations must be made 24 hours in advance.
